How to Customized Subscription Form on your Blog/websites(Enter your email address form)

A colleague blogger emailed me to teach him how he could customized subscription form on his blog so that the subscription form could be delivered by his blog/website's name . Customising some elements on your website/blog tells how professional you at your blogging. One of the importance of customisation is attract and sustain visitors interest for possible coming back.
 After you'v gotten your code must do some editing.

1. Copy and paste the code below on a note pad
2.Change the codes in RED ink
3.Put you blog/ website domain name in place of RED ink
4. Click save.
